Epic Health Services is a Medicare/Medicaid certified home health care agency with a community of nurses, rehab professionals, and paraprofessionals dedicated to providing the best quality of care to patients at home on a one-to-one basis. We are seeking qualified psychiatric and medical/surgical home care nurses to handle our growing business. Assignments include: Provide skilled nursing care to patients in a home setting Assess home health patients to identify the physical, psycho-social, and environmental needs Implement/develop/document the plan of care to ensure quality and continuity of care for patients Educating patients and caregivers Collaborative communication between patient/family, teammates, and other health care professionals Appropriate reporting of incidents, injuries, infection, and all other components of risk management Attends and participates in staff meetings, required in-services, and other meetings as requested Job responsibilities:
